Based on your "guild" or "clan's" level.
Your guild will be assigned a instance, your guild has a limited time frame to complete this instance.
Depending on your guild's level will depend which "group" of instances you may be assigned from, and how many assignments you will be allotted. The lower level and smaller instances will have a shorter time frame for completion and once completed your guild will automatically receive a new assignment.
The larger instances will require a guild with a higher level. You will be given significantly more time to complete it. Example: Your guild has been assigned to Dungeon X. You have 14 days to complete dungeon X. If you complete it within 7 days your completion reward is doubled and you'll be awarded a new instance at the next instance rollover.
Instances will range from "solo" to small group 3-5 people, up to 40+ people.
Instances will be "randomly" generated. This will be done from preform "blocks" that the server snaps together, to form a complete instance.
